365Backgroundchecks, LLC, doing business as 365 EasyScreen (“365 EasyScreen,” “we,” “us,” or “our”), provides drug and alcohol testing, occupational health, and DOT compliance support services to employers. This Privacy Policy explains what information we collect through this website and in the course of providing our services, how we use it, and the choices available to you.
Information We Collect
Depending on how you interact with 365 EasyScreen, we may collect:
- Website visitor information — name, company, email, and phone number submitted through our contact and quote-request forms.
- Employer/client account information — billing contact, company details, and program configuration submitted by our employer clients.
- Donor/employee information — name, date of birth, and identifying information required for testing, collection, and chain-of-custody records, submitted by our employer clients or their designated collection sites in connection with a specific testing event.
- Testing and program records — specimen collection records, chain-of-custody documentation, laboratory and Medical Review Officer (MRO) results, and, where applicable, FMCSA Clearinghouse query results, processed through the Abbott eScreen network on behalf of our employer clients.
- Technical information — standard website usage data (pages visited, browser type) collected automatically when you use this site.
How We Use Information
- To provide, schedule, and administer drug and alcohol testing, occupational health services, and DOT compliance support requested by our employer clients.
- To communicate with employer clients and website visitors about services, quotes, and account matters.
- To meet our obligations as a registered C/TPA under the FMCSA Clearinghouse and other applicable DOT and state requirements.
- To maintain the audit trail and documentation required to support our clients' testing programs.
365 EasyScreen does not use donor/employee testing data for marketing or advertising purposes. Limited website-visitor data may be used for advertising retargeting as described below; see How Information Is Shared and Your Privacy Choices.
How Information Is Shared
We share information only as needed to deliver the service:
- With the Abbott eScreen network and its affiliated collection sites, certified laboratories, and Medical Review Officers, to process and report test results.
- With the employer client on whose behalf a test or service was requested.
- With the FMCSA Clearinghouse, where required by 49 CFR Part 382, Subpart G.
- With service providers who support our operations (for example, our client portal), under confidentiality obligations.
- As required by law, subpoena, or valid legal process.
We share limited website-visitor information (such as pages viewed and device or browser identifiers) with Meta and Google through their advertising tools, so we can show ads to past visitors of this site and measure ad performance. Under the CCPA/CPRA, this is considered “sharing” personal information for cross-context behavioral advertising, even though we do not sell information and receive no payment for it. You can opt out at any time; see Your Privacy Choices below. We do not use donor/employee testing data for this purpose.
Sensitive Information
Drug and alcohol test results and related program records may be considered sensitive or health-adjacent personal information under some state privacy laws. We limit our use of this information to the testing, compliance, and program-administration purposes described above, and do not use it for marketing or profiling.
Cookies & Tracking Technologies
This site uses standard web technologies, including a Meta (Facebook) pixel and Google advertising tags, for site traffic measurement, ad performance measurement, and retargeting or audience-building (showing ads to people who have previously visited this site). These tools may share limited visitor information with Meta and Google for that purpose; see How Information Is Shared above and Your Privacy Choices below. These tools do not run on any page where donor or employee testing information is submitted or viewed, including scheduling, results, and dispute pages. You can control cookies through your browser settings and can opt out of retargeting through Your Privacy Choices.
Your Privacy Choices
You have the right to opt out of this sharing. To opt out, use the “Your Privacy Choices” link in the site footer, or set the Global Privacy Control (GPC) signal in your browser — we treat a GPC signal as a valid opt-out request automatically, without requiring you to also click the link. Opting out does not affect our ability to provide services to you or your employer. This choice does not apply to donor/employee testing data, which is never used for advertising.
Geographic Scope
This site and this Privacy Policy are directed to visitors and clients in the United States. We do not knowingly direct our services to individuals outside the United States.
Data Retention
We retain testing and program records for the periods required under 49 CFR § 40.333: five years for verified positive drug results, alcohol results of 0.02 or greater, refusals to test (including substituted or adulterated results), SAP reports, and follow-up test records; three years for prior-employer test result information obtained under 49 CFR § 40.25; two years for EBT calibration and maintenance records; and one year for negative and cancelled test results. Records are securely deleted or de-identified once the applicable retention period expires.
Your Rights
If you are a California resident, you have the right under the CCPA/CPRA to opt out of the sharing of personal information described above (see Your Privacy Choices), as well as rights to know, access, correct, or delete certain personal information we hold about you. We will respond to a verifiable request within 45 days of receipt, extendable by an additional 45 days when reasonably necessary, with notice of any extension. Residents of Virginia, Colorado, Connecticut, Utah, and other states with comprehensive privacy laws have similar rights under their state's law and response timelines. Because much of the information we process is collected and controlled by your employer for a specific testing or compliance purpose, requests regarding donor/employee testing records should generally be directed to your employer in the first instance. You may also contact us directly using the information below.
Security
See our Security & Data Handling page for how we protect information in our care.
Children's Privacy
This site and our services are directed to employers and their workforce and are not intended for individuals under 18.
Changes to This Policy
We may update this Privacy Policy from time to time. The “Last Updated” date above reflects the most recent revision.
